What model phone do I have?

From the home screen on your phone, swipe up or tap All Apps. Tap the Settings app to open it. It's the app with a gear icon (if you don't see it, tap the search bar at the top of the screen and type in Settings). In the Settings app, tap on About Phone. You will see the phone's model name displayed at the top
